Required documents for claim registration:
Completed ADNIC Claim Form OR Medical report containing the diagnosis, onset and duration of illness, visit date and treatment details with Dr. Stamp and signature.
Receipts with itemized breakdown.
Valid prescription:
Lab Reports/Lab Referral (If applicable).
Physiotherapy/Alternate medicine Referral (If applicable).
Admission and discharge summary with itemized breakdown (for Hospitalization).
Prior approval for overseas elective treatment (subject to policy terms).
RTA report if treatment is due to road traffic accident.
Note:
The initial claim submission time period is 120 days from treatment incurred date and time period for resubmission of additional documents if requested is 30 days from the date of intimation. Claim settlement is subject to policy terms and conditions.
Non-medical expenses like taxes, medical report charges, telephone charges etc are excluded.
Additional documents apart from those listed above may be requested if deemed necessary for claim evaluation.
All the documents including invoices and medical reports should be in either English or Arabic.
Please retain original documents for a period of 1 year from incurred date for ADNIC audit purpose.
If the total claim amount is more than AED 5,000, please visit any of our branches or courier the original receipts/documents.

I hereby declare that the information above is true and complete and that reimbursement requested is for expenses paid by me/ my dependent for the treatment of my/ my dependent's medical condition.
I agree to submit to ADNIC any requested document mandatory/ deemed necessary to process my above claim. I hereby authorize ADNIC to approach any doctor / medical facility / any Institution or any person who has any record / medical information about me or my family member to provide ADNIC with complete information including copies of the records when requested.
